I solemnly swear that I was born Bradford, England, in the State of [blank], on or about the 22 day of June, 1886,

that my husband Frederick C Rawson, was born in Bradford England and is now residing at Braintree Mass.

that he emigrated to the United States from the port of Liverpool, on or about Feb 9 1909;

that he resided 11 years, uninterruptedly, in the United States, from 1909 to 1920; at Winthrop Boston;

that he was naturalized as a citizen of the United States before the District Court of U S, at Boston Mass, on April 1, 1918 ???? of the person attached accompanying Certificate of Naturalization;

that I have resided inside the United States at the following places for the following periods:

and that I am domiciled in the United States, my permanent residence being at Braintree, in the State of Mass, where I follow the occupation of None

My last passport was obtained from None, on - and was [blank] I am about to go abroad temporarily; and I intend to return to the United States within 3 months with the purpose of residing and performing the duties of citizenship therein;

and I desire a passport for use in visiting the countries hereinafter named for the following purpose:

I intend to leave the United States from the port of New York sailing on board the not yet known on early May, 1920
